Your customer database is arguably your most important asset. It should not be treated with anything less than the highest level of importance
Most important things here are: 1. Knowing which records need updation 2. Knowing who is supposed to update which record. If this is not possible YOU could assign these responsibilities 3. Give out clear, firm, instructions with deadlines 4. Better still, ask THEM what can be done to solve this problem and how can they help
If no employee complies, irrespective of how good he is, then something is fundamentally wrong about it and needs some change at a fundamental level. Whereas if only some employees dont comply, then TAKE a look at other aspects of each of their performance. This could reveal some valuable insights. It can help you root out some deadwood.

Your posting is a bit confusing. Yes, client data updation is important, but it is more important to protect it. Client data should be accessible to very few people, only the top rung. If you mean the doctors' data, yes, it needs to be updated. As for rooting out deadwood, we don't need the subterfuge to remove inefficient or undisciplined staff.
